PLAN & LOGIC MODEL ASSIGNMENT
What is the assignment?
For this assignment, you will need to select a human services organization: community-based, non-profit, faith-based or
public sector organization. Then, develop, describe, and analyze a logic model (LM) for a specific program/service with a
corresponding performance/outcome measurement plan. The focus of the LM must be a program / intervention that
actively engages the target population of the organization whose outcome is a behavioral and/or change of condition.
You are encouraged to supplement the assignment with supporting documents from the organization. You may also want
to make contact with the organization to align information in the LM.
It is highly recommended to use the website listed below to build your logic model. However, you may use Microsoft
Word (and its chart features) or Excel. When necessary and appropriate, be sure to align items in the logic model so
information can be easily tracked from inputs through impacts. Appendices might include narrative/notes to clarify
specific parts of the model and operational definitions for terms. You should also provide three citations to reference you
selection of logic model style and components (example: the theory of change section should cite resources and include
resources in a reference listing).
How is this assignment related to your future career?
As a human services professional, you should full understand how to build and evaluate the programs
you are planning to, or have already implemented.
Critical reflection and thinking is a skill which is required in your field. This project allows for practice
of this skill.

Name and purpose of the organization and program for whom the LM is being developed, including the

The specific target population and service delivery area for the program.
The theory of change for the strategies / programs / interventions being proposed. This is not an opinion;
the theory of change should reflect an evidence-based framework, theory, or model with references /
